Epidemiological factors predicting breast cancer survival in Russia have not been studied enough.</p><p><bold>   Aim. </bold>To evaluate the factors predicting survival in BC by the data of the Arkhangelsk Regional Cancer Registry in 2000–2020 and the survival during the COVID-19 pandemic</p><p><bold>   Materials and methods. </bold>Data on all cases of Вс for 2000–2020 retrieved from the Arkhangelsk Regional Cancer Registry database. Cancer-specific survival was estimated using the Kaplan–Meier and the life tables method. The prognostic value of variables available in Arkhangelsk Regional Cancer Registry was assessed in Cox regression.</p><p><bold>   Results. </bold>The final analysis included 9,089 cases of breast cancer. During the observation period, 2723 deaths from breast cancer were registered. The one-, five- and ten-year survival rates in the entire population were 91.6 %, 73.0 % and 64.0 %, respectively. In a multivariate analysis, the risk of death from BC increased by 2.9, 8.2, 21.3 and 6.8 at 2, 3, 4 and unknown stages; 2.1 times with the localization of the tumor in the area of the nipple and areola; by 33 % in ductal cancer compared with lobular cancer, by 1.8 and 2.6 times in those who received palliative treatment and did not receive treatment; by 33 % in women aged 80 years and older; by 22 % for those living in rural areas. In the period 2013–2019, the 5-year cancer-specific survival reached 79.1 %, which is 14 % and 6 % higher than in the periods 2000–2006 and 2007–2012. Survival rates have not declined during the COVID-19 pandemic.</p><p><bold>   Conclusion. </bold>The risk of death from BC is increased in advanced stages, ductal morphology, localization in the nipple-areolar region, age over 80 years, and living in rural areas. During the COVID-19 pandemic BC survival did not worsen, but this requires further study.</p></abstract><trans-abstract xml:lang="ru"><p><bold>   Введение. </bold>Рак молочной железы (РмЖ) лидирует в структуре заболеваемости злокачественными новообразованиями в мире и России. Эпидемиологические факторы прогноза выживаемости РмЖ в России изучены недостаточно.</p><p><bold>   Цель исследования</bold> – оценить факторы прогноза выживаемости при РмЖ по данным архангельского областного канцер-регистра за 2000–2020 гг. и выживаемость в период пандемии COVID-19.</p><p><bold>   Материалы и методы. </bold>Сведения обо всех случаях РмЖ за 2000–2020 гг. извлечены из базы данных архангельского областного канцер-регистра. Оценивали опухолеспецифическую выживаемость с помощью методов Капланамейера и метода life tables. степень влияния на прогноз отдельных факторов, доступных в базе данных регистра, оценивали в регрессии Кокса.</p><p><bold>   Результаты.</bold> В итоговый анализ вошло 9089 случаев РмЖ. за период наблюдения зарегистрировано 2723 смерти от РмЖ. Показатели 1-, 5- и 10-летней выживаемости во всей популяции составили 91,6; 73,0 и 64,0 % соответственно. Во множественной регрессионной модели риск смерти от РмЖ повышался в 2,9; 8,2; 21,3 и 6,8 раза при II, III, IV и неизвестной стадии; в 2,1 раза при локализации опухоли в области соска и ареолы; на 33 % при протоковом раке сравнительно с дольковым; в 1,8 и 2,6 раза у получавших паллиативное лечение и не получавших лечения; на 33 % у женщин в возрасте 80 лет и старше; на 22 % у проживающих в сельской местности. В период 2013–2019 гг. 5-летняя опухолеспецифическая выживаемость достигла 79,1 %, что на 14 и 6 % выше, чем в 2000–2006 и 2007–2012 гг. В период пандемии COVID-19 показатели выживаемости не снижались.</p><p><bold>   Выводы.</bold> Риск смерти от РмЖ повышен при распространенных стадиях, протоковой морфологии, локализации опухоли в сосково-ареолярной области, возрасте старше 80 лет и проживании в сельской местности.
