We also performed histological and immunohistochemical examination of tumor tissue samples from breast cancer patients (for the retrospective cohort study) and statistical data analysis. A total of 1,216 patients with T1–2N0M0 breast cancer were included in this study. Histological and immunohistochemical examinations of tissue samples (paraffin blocks) were conducted in the laboratory of N. N. Petrov National Medical Research Center of Oncology. We stained slides for both routinely used markers (including estrogen receptors, progesterone receptors, HER2, and Ki-67) and other markers (CK14, FOXA1, FOXP3, PD-L1, P53, SMA, androgen receptors, E-cadherin, CD4, CD8, CK5 / 6, EGFR).We analyzed risk factors for lethal outcomes in patients from this group to develop prognostic scales and compared their results.</p> <p><bold> Results.</bold> We evaluated the most clinically and statistically significant factors affecting mortality. Using logistic regression, we chose 10 factors that had the greatest impact on the outcomes and then produced several scales, includinga 10-point regression scale (based on 10 most significant factors identified). Survival analysis in high-risk and low-risk patients using the regression scale demonstrated significant differences between these groups (р &lt;0.00001). The assessment of adjuvant chemotherapy efficacy in the combined group of intermediate- and high-risk patients (as estimated by the regression model) showed that intermediate- and high-risk patients receiving adjuvant chemotherapy had significant differences in their survival (р = 0.0057). The regression scale for 10-year prognosis demonstrated sufficient sensitivity (58.05 %), specificity (69.47 %) and ef fectiveness (63.76 %).</p> <p><bold> Conclusion. </bold>Our regression prognostic scale includes markers with a high prognostic value. The multifactorial approach used in the developed regression scale for breast cancer 10-year prognosis increases its accuracy and reliability.</p></abstract><trans-abstract xml:lang="ru"><p><bold> Введение.</bold> Рак молочной железы (РмЖ) – наиболее часто встречающееся онкологическое заболевание в женской популяции. за последние десятилетия смертность от РмЖ в мире несколько снизилась благодаря широкому внедрению маммографического скрининга, адъювантной и неоадъювантной системной терапии. Однако адъювантная химиотерапия в настоящее время уже не является общепринятым стандартом лечения. Современные методики анализа молекулярно-генетического профиля опухоли позволяют выделить группу благоприятного прогноза среди больных HER2– РмЖ с наличием экспрессии эстрогеновых рецепторов, которые не нуждаются в адъювантной химиотерапии. Несмотря на сложности внедрения молекулярно-генетического профилирования опухоли в рутинную практику, суррогатное определение подтипов РмЖ на основании оценки уровня экспреcсии эстрогеновых, прогестероновых рецепторов, HER2, Ki-67 эффективно с точки зрения формирования индивидуального прогноза и выбора адъювантного лечения. Все более актуальным становится изучение биологических маркеров РмЖ, являющихся альтернативой стандартным маркерам, широко применяемым в рутинной клинической практике. Целый ряд показателей, таких как р53, CK5 / 6, SMA, p63, PHH3, е-кадгерин, EGFR, FOXA1, рецепторы андрогенов, TILs и др., в многочисленных исследованиях демонстрируют предиктивную и / или прогностическую значимость. Результаты подобных исследований в будущем могут существенно повлиять на формирование новых подходов к лечению РмЖ.</p> <p><bold> Цель исследования</bold> – повысить эффективность системной терапии РмЖ, снизить количество необоснованных назначений, используя данные об индивидуальных иммуногистохимических характеристиках опухоли, и разработать прогностические шкалы для обеспечения персонализированного подхода к назначению адъювантного системного лечения РмЖ.</p> <p><bold> Материалы и методы. </bold>Проведено комплексное многоплановое исследование, включавшее сбор данных литературы о клинических, патоморфологических, прогностических и предиктивных факторах РмЖ, а также ретроспективное когортное исследование данных канцер-регистра, гистологические, иммуногистохимические исследования образцов опухолевой ткани больных РмЖ (ретроспективное когортное исследование), статистический анализ данных. Общая численность исследуемой популяции составила 1216 больных РмЖ T1–2N0M0. Гистологические и иммуногистохимические исследования были выполнены в лаборатории фгбу «Национальный медицинский исследовательский центр онкологии им. Н. Н. Петрова» минздрава России с использованием архивного материала – парафиновых блоков опухолей исследуемой группы. Для иммуногистохимических исследований архивный материал был окрашен антителами как к рутинно использующимся маркерам (эстрогеновые, прогестероновые рецепторы, HER2, Ki-67), так и к изучаемым в настоящее время (CK14, FOXA1, FOXP3, PD-L1, P53, SMA, андрогеновые рецепторы, е-кадгерин, CD4, CD8, CK5 / 6, EGFR). Проанализированы факторы риска летального исхода у пациентов данной группы с целью разработки прогностических шкал, проведено сравнение полученных результатов.</p> <p><bold> Результаты. </bold>Оценены наиболее значимые с клинической и статистической точек зрения факторы, влияющие на летальный исход, из числа которых на основе логистической регрессии выбраны 10 факторов, оказывающих наибольшее влияние на данную переменную. На основе полученных результатов построены балльные шкалы, в том числе регрессионная шкала, включающая 10 соответствующих факторов. Анализ выживаемости пациенток групп высокого и низкого риска по регрессионной шкале продемонстрировал выраженные статистически значимые различия (р &lt;0,00001). Оценка эффективности адъювантной химиотерапии в объединенной группе среднего и высокого риска летального исхода по регрессионной шкале показала, что пациентки групп среднего и высокого рисков, получающие адъювантную химиотерапию, имели статистически значимые отличия в выживаемости (р = 0,0057). Результаты применения регрессионной шкалы для показателя 10-летнего прогноза продемонстрировали достаточную чувствительность (58,05 %), специфичность (69,47 %) и эффективность (63,76 %).</p> <p><bold> Заключение.</bold> Разработанная регрессионная прогностическая шкала включает маркеры, для которых была выявлена высокая прогностическая значимость.
