Over the past 4 decades, the HIV / AIDS pandemic has been considered as one of the world’s most serious public health problems. Globally, there were approximately 38 million people living with HIV in 2022; 53 % of them were women. The number of HIV-infected patients living in Russia reaches 1,168,000. The incidence and prevalence of HIV infectionvaries across different regions of Russia. High prevalence of cervical cancer usually correlates with HIV prevalence, which is associated with the long-term and aggressive persistence of human papillomavirus (HPV) in HIV patients. HIV co-infection is believed to be the main risk factor for HPV-related precancerous lesions to the cervical epithelium and their rapid progression to invasive cancer.</p><p><bold>   Aim.</bold> To analyze risk factors for poor prognosis in HIV-infected patients with HPV.</p><p><bold>   Materials and methods.</bold> This study included 15 HIV-infected patients with cervical cancer treated in Novosibirsk Regional Clinical Oncology Dispensary or Federal Research and Clinical Center for Specialized Medical Care and Medical Technologies, Federal Biomedical Agency of Russia between 2016 and 2021.</p><p><bold>   Results. </bold>Median age of the patients was 38 years; all of them were HPV-positive. Their viral load was &gt;400 copies / mL and baseline CD4+ cell count ≥350 cells / mL. All patients were diagnosed with AIDS, given the presence of HPV infection. During the therapy for cervical cancer, patients also received combination antiretroviral treatment, including Lamivudine Advanced at a dose of 300 mg / day. Ten patients received comprehensive treatment, while 8 patients received chemoradiotherapy.</p><p><bold>   Conclusion. </bold>Given the more aggressive course of cervical cancer in HIV-infected patients, their treatment plan should be developed by a multidisciplinary team, including an infectious disease specialist, clinical pharmacologist, radiotherapist, and gynecological oncologist. Such patients should receive maximal antitumor treatment, as well as intensive antiretroviral therapy. HIV-positive women with cervical cancer require special attention to ensure treatment completion and to optimize long-term outcomes.</p></abstract><trans-abstract xml:lang="ru"><p><bold>   Введение. </bold>Инфекция, вызванная вирусом иммунодефицита человека (ВИЧ), – антропонозная ретровирусная инфекция с медленным развитием, длительным течением и разнообразными клиническими проявлениями, которая завершается развитием синдрома приобретенного иммунодефицита (СПИД). За последние 4 десятилетия пандемия ВИЧ / СПИДа признана одной из самых серьезных мировых проблем общественного здравоохранения. По оценкам, в 2022 г. во всем мире насчитывается около 38 млн человек, живущих с ВИЧ, 53 % составляют женщины. На долю России приходится 1 168 000 больных. Заболеваемость и распространенность ВИЧ-инфекции по регионам характеризуются неоднородностью. Регионы высокой распространенности рака шейки матки (РШМ) соответствуют регионам высокой распространенности Вич-инфекции, что связано с длительной и агрессивной персистенцией вируса папилломы человека у инфицированных пациенток. Коинфекция ВИЧ является основным фактором риска развития предраковых поражений эпителия шейки матки, вызванных вирусом папилломы человека, и быстрого их прогрессирования до инвазивного рака.</p><p><bold>   Цель исследования </bold>– современная оценка факторов риска неблагоприятного прогноза у больных РШМ с ВИЧ-инфекцией.</p><p><bold>   Материалы и методы. </bold>Нами проанализированы 15 клинических наблюдений больных РШМ с ВИЧ-инфекцией, проходивших лечение в ГБУЗ НО «Новосибирский областной клинический онкологический диспансер» или ФГБУ «Федеральный научно-клинический центр специализированных видов медицинской помощи и медицинскихтехнологий федерального медико-биологического агентства России» в период 2016–2021 гг.</p><p><bold>   Результаты.</bold> В результате выполненного анализа мы обнаружили, что средний возраст пациенток составлял 38 лет, все пациентки имели ВПЧ-положительный статус, с учетом наличия ВИЧ-инфекции и плоскоклеточного РШМ рассматривались как больные СПИДДом, имели вирусную нагрузку РНК более 400 копий / мл и начальное количествоCD4 не менее 350 клеток / мл. Во время лечения РШМ все пациентки получали современную комбинированную ретровирусную терапию, включая ламивудин-Эдвансд 300 мг / сут. 10 пациенток получили комплексное лечение, 8 – химиолучевую терапию.</p><p><bold>   Выводы. </bold>С учетом более агрессивного течения РШМ у женщин с ВИЧ-инфекцией планирование лечения должна проводить многопрофильная команда, обязательно включающая инфекциониста, клинического фармаколога, радиотерапевта, онкогинеколога. Противоопухолевое лечение следует назначать в максимальном объеме в зависимости от стадии заболевания, с интенсивной ретровирусной терапией.
