Petrov Research Institute of Oncology. The average body mass index was 29.69 ± 3.92 kg/m<sup>2</sup>. Obesity was diagnosed in 86.4 % (19/22) of cases. All patients had 2 or 3 degrees of mammary gland ptosis.</p><p><bold>Results. </bold>In 59.1 % (13/22) of cases patients underwent unilateral Goldilocks mastectomies, in 40.9 % (9/22) – bilateral operations using this technique. In 16 patients (72.7 %) the nipple-areolar complex was preserved. The overall complication rate was 12.9 % (4/31). The patients noted that the final subjective perception of the treatment results exceeded the expectations of the performed surgical intervention.</p><p><bold>Conclusion. </bold>Breast reconstruction in overweight patients can be performed in various ways. Based on the experience of the N.N. Petrov Research Institute of Oncology and the results of this study, Goldilocks mastectomy is the best reconstruction option for patients with high body mass index or ptosis of the mammary glands, as well as concomitant diseases. This technique allows the surgeon to remove breast tissue within the anterior and posterior leaves of the superficial fascia, create additional volume using a de-epidermized flap, which preserves oncological safety and improves the aesthetic result.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. М</bold>астэктомия по типу Goldilock – это инновационная хирургическая техника, разработанная для пациенток с диагнозом рака молочной железы, которые ищут баланс между онкологически безопасным удалением опухоли и оптимальным окончательным эстетическим результатом.</p><p><bold>Цель исследования </bold>– выполнение оптимального типа реконструкции у пациенток с избыточной массой тела и серьезными сопутствующими заболеваниями, отказавшихся от стандартных вариантов восстановления формы молочных желез, путем сохранения деэпидермизированного нижнего лоскута во время мастэктомии.</p><p><bold>Материалы и методы.</bold> На базе Национального медицинского исследовательского центра онкологии им. Н.Н. Петрова в период с июня 2023 г. по ноябрь 2024 г. выполнено 31 оперативное вмешательство в объеме мастэктомии по типу Goldilock у 22 пациенток в возрасте от 31 до 75 лет. средний индекс массы тела составил 29,69 ± 3,92 кг/м<sup>2</sup>. Ожирение было диагностировано в 19 (86,4 %) из 22 случаев. у всех пациенток наблюдался птоз молочной железы II или III степени.</p><p><bold>Результаты.</bold> В 13 (59,1 %) из 22 случаев пациенткам были выполнены односторонние мастэктомии по типу Goldilock, в 9 (40,9 %) из 22 – билатеральные операции по данной методике. у 16 (72,7 %) пациенток удалось сохранить сосково-ареолярный комплекс. Общая частота осложнений составила 12,9 % (4 случая из 31). Пациентки отметили, что, по субъективному восприятию, окончательные результаты лечения превзошли ожидания от выполненного оперативного вмешательства.</p><p><bold>Выводы.</bold> Реконструкцию молочных желез у пациенток с избыточной массой тела можно выполнить разными способами. На основании опыта Национального медицинского исследовательского центра онкологии им. Н.Н. Петрова и результатов данного исследования мастэктомия по типу Goldilock является оптимальным вариантом реконструкции для пациенток с высоким индексом массы тела или птозом молочных желез, а также сопутствующими заболеваниями.
