To evaluate the efficacy of interstitial high-dose rate brachytherapy (HDRB) or irradiation by electron as the methods for delivering boost to the tumor bed.</p><p><bold>Materials and methods.</bold> We performed a retrospective analysis of 5-year local control after whole breast irradiation and additional boost delivery to the tumor bed by HDRB or by electrons. In 485 breast cancer patients included in the analysis HDRB boost was performed in 247, by electrons – in another 238 women.</p><p><bold>Results</bold>. In 485 evaluated patients, local recurrences were diagnosed in 22 (4.5 %) cases: in 10 (4.0 %) of 247 women after HDRB boost and in 12 (5.0 %) of 238 patients who received a boost by electrons (р = 0.536). We found that age less than 50 years and aggressive subtypes (triple-negative, HER2+, luminal B HER2+) were associated with an increased risk of local recurrence (р = 0.003 and р = 0.004, respectively). The type of boost delivery (HDRB vs. electrons) does not influence the probability of local control in young women (р = 0.547) or in patients with aggressive subtypes of breast cancer. Conclusion. Both techniques of boost delivery (HDRB or electrons) are equally effective in prevention of local recurrence.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – сравнение эффективности дополнительного облучения ложа удаленной опухоли молочной железы (ЛУОМЖ) с помощью брахитерапии источниками высокой мощности дозы и дистанционного облучения электронами.</p><p><bold>Материалы и методы</bold>. Проведен ретроспективный анализ показателей 5-летнего локального контроля у 485 больных раком молочной железы, которым после адъювантной лучевой терапии осуществлялось дополнительное облучение ЛУОМЖ. Все пациентки были разделены на 2 группы в зависимости от способа подведения дозы: в 1-ю группу были включены 247 женщин, которым дополнительное облучение ЛУОМЖ проводилось с помощью внутритканевой высокодозной брахитерапии, во 2-ю – 238 больных, которым дополнительная доза на ЛУОМЖ подводилась с помощью облучения электронами.</p><p><bold>Результаты</bold>. Локальные рецидивы диагностированы у 22 (4,5 %) из 485 пациенток, включенных в исследование. При проведении дополнительного облучения ЛУОМЖ с помощью высокодозной брахитерапии локальные рецидивы выявлены у 10 (4,0 %) из 247 пролеченных пациенток, с помощью электронов – у 12 (5,0 %) из 238. Различия в показателях локального контроля между группами статистически недостоверны (р = 0,536). Проведенный сравнительный анализ показал достоверное увеличение частоты локальных рецидивов у женщин с агрессивными подтипами (трижды негативный, люминальный В HER2+, HER2+) в сравнении с больными раком молочной железы «благоприятных» подтипов (люминальный А, люминальный В): 9,3 и 2,4 % соответственно (р = 0,003). Достоверное повышение частоты локальных рецидивов обнаружено и в группе женщин моложе 50 лет (7,7 %) в сравнении с женщинами старше 50 лет (2,2 %) (р = 0,004). При анализе влияния технологии дополнительного облучения ЛУОМЖ в группах с повышенным риском возникновения локальных рецидивов достоверных различий в частоте локальных рецидивов не выявлено (р = 0,547 и 0,798 соответственно).</p><p><bold>Выводы</bold>.
