Intraoperative tumor capsule rupture and the presence of tumor cells in ascites / peritoneal washings are considered potentially significant outcome predictors.</p> <p><bold>Aim.</bold> To evaluate the impact of tumor capsule rupture, ascites, and tumor cells in peritoneal fluid on overall survival (OS) and progression-free survival (PFS) in patients with stage I–II OC.</p> <p><bold>Materials and methods.</bold> A retrospective analysis of data from 209 patients with stage I–II OC (FIGO, 2016) treated between 1990 and 2019 was conducted. Tumor capsule rupture was detected in 67 (32.0 %) patients, ascites – in 25 (12.0 %) patients. Cytological examination of ascitic fluid or peritoneal washings was performed in 80 patients; tumor cells were found in 29 (36.3 %) of them. Statistical analysis was performed using the Kaplan–Meier method, <italic>log-rank</italic> test, and Cox regression.</p> <p><bold>Results.</bold> Tumor capsule rupture was associated with a statistically significant decrease in survival rates: 5-year OS was 66.8 % <italic>vs</italic>. 87.6 % (<italic>p</italic> = 0.032), and 5-year PFS was 53.1 % <italic>vs</italic>. 73.4 % (<italic>p</italic> = 0.037) in groups with and without tumor capsule rupture, respectively. The hazard ratio (HR) for OS was 0.61 (95 % confidence interval (CI) 0.39–0.96). The presence of ascites did not show a significant effect on OS (HR 1.11; 95 % CI 0.70–1.77) and PFS (HR 0.96; 95 % CI 0.65–1.42). The detection of tumor cells in peritoneal fluid significantly worsened PFS: 5-year PFS was 63.1 % <italic>vs</italic>. 85.7 % (<italic>p</italic> = 0.020) in case of their presence and absence, respectively (HR 2.49; 95 % CI 0.86–7.21).</p> <p><bold>Conclusion.</bold> Tumor capsule rupture and the presence of tumor cells in ascitic fluid / peritoneal washings are significant adverse prognostic factors in patients with stage I–II OC. The obtained data emphasize the importance of a careful surgical approach to prevent tumor capsule rupture and the necessity of a thorough cytological examination of ascitic fluid and peritoneal washings to determine the risk of death and progression in stage I–II OC.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Рак яичников (РЯ) характеризуется высокой летальностью, что определяет актуальность изучения прогностических факторов на ранних стадиях заболевания. Нарушение целостности капсулы опухоли и наличие опухолевых клеток в асцитической жидкости / смывах с брюшины рассматриваются как потенциально значимые предикторы прогноза жизни пациентки и прогрессирования РЯ.</p> <p><bold>Цель исследования</bold> – оценить влияние нарушения целостности капсулы опухоли, асцита и наличия опухолевых клеток в перитонеальной жидкости на общую выживаемость (ОВ) и выживаемость без прогрессирования (ВБП) заболевания у пациенток с РЯ I–II стадий.</p> <p><bold>Материалы и методы.</bold> Проведено ретроспективное исследование данных 209 больных РЯ I–II стадий (FIGO, 2016), получивших лечение в период с 1990 по 2019 г. Нарушение целостности капсулы опухоли выявлено у 67 (32,0 %) пациенток, асцит – у 25 (12,0 %). Цитологическое исследование асцитической жидкости или перитонеальных смывов выполнено 80 пациенткам; у 29 (36,3 %) из них обнаружены опухолевые клетки. Статистический анализ проводился с использованием методов Каплана–Мейера, <italic>log</italic><italic>-</italic><italic>rank</italic> и регрессии Кокса.</p> <p><bold>Результаты.</bold> Нарушение целостности капсулы опухоли ассоциировалось со статистически значимым снижением выживаемости: 5-летняя ОВ составила 66,8 ± 6,3 % против 87,6 ± 2,9 % (<italic>p</italic> = 0,032), 5-летняя ВБП – 53,1 ± 6,5 % против 73,4 ± 3,9 % (<italic>p</italic> = 0,037) в группах с нарушением целостности капсулы опухоли и без него соответственно. Отношение рисков (ОР) для ОВ составило 0,61 (95 % доверительный интервал (ДИ) 0,39–0,96). Наличие асцита не оказало значимого влияния на ОВ (ОР 1,11; 95 % ДИ 0,70–1,77) и ВБП (ОР 0,96; 95 % ДИ 0,65–1,42). Обнаружение опухолевых клеток в асцитической жидкости или перитонеальных смывах значимо ухудшало ВБП: 5-летняя ВБП составила 63,1 ± 10,0 % против 85,7 ± 7,6 % (<italic>p</italic> = 0,020) при их наличии и отсутствии соответственно (ОР 2,49; 95 % ДИ 0,86–7,21).</p> <p><bold>Выводы.</bold> Нарушение целостности капсулы опухоли и наличие опухолевых клеток в асцитической жидкости / смывах с брюшины являются значимыми неблагоприятными прогностическими факторами у больных РЯ I–II стадий.
