Expression of survivin was most often determined in medium and high grade ductal carcinoma (G2–G3), and was associated with lymphovenous stromal invasion in most cases. Expression of survivin protein correlated with the expression of HER2/neu. In 59.6 % of cases survivin was expressed in tumors with a low Ki-67 index. Most often, survivin was expressed in luminal A and luminal B molecular-biological tumor subtypes. Real-time polymerase chain reaction determined the expression of BIRC5 gene in all 67 carcinoma samples. The level of normalized expression of BIRC5 gene significantly moderately correlated with the expression of its product – survivin protein (r = 0.704, p &lt;0.01) and slightly correlated with the expression of oncoprotein HER2/neu (r = –0.285, p &lt;0.05). The median follow-up was 40 months. Overall survival of patients without survivin expression in primary tumor was 100 %, overall survival of patients with survivin expression was 77.16 % (range 74.6–80.1 %) (p = 0.041), relapse-free survival was 100 % and 71.9 % (range 70.1–74.2 %) respectively (p = 0.037). The ratio of progression risks in the survivin expression group was 10.6 (95 % confidence interval 0.85–132.2) (p = 0.041).</p><p><bold>Conclusion. </bold>Expression of the protein survivin and its gene BIRC5 is an independent adverse prognostic factor and can be used as a predictive and prognostic marker.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – изучить экспрессию антиапоптотического протеина сурвивина и кодирующего его гена BIRC5 в первичной карциноме молочной железы как потенциальный прогностический и предиктивный маркер.</p><p><bold>Материалы и методы</bold>. При помощи методов иммуногистохимии и полимеразной цепной реакции в режиме реального времени изучено 67 образцов (биоптатов) первичной карциномы молочной железы на предмет наличия экспрессии белка сурвивина и кодирующего его гена BIRC5.</p><p><bold>Результаты</bold>. Экспрессия сурвивина определялась в 47 (70,15 %) образцах карциномы молочной железы, наиболее часто – в протоковой карциноме средней и высокой степени злокачественности (G2–G3), и в большинстве случаев сочеталась с лимфовенозной стромальной инвазией. Экспрессия белка сурвивина коррелировала с экспрессией HER2/neu. В 59,6 % случаев сурвивин экспрессировался в опухолях с низким уровнем Ki-67. Наиболее часто сурвивин экспрессировался при люминальном А и люминальном B молекулярно-биологических подтипах опухоли. Полимеразная цепная реакция в режиме реального времени определила экспрессию гена BIRC5 во всех 67 образцах карциномы. Уровень нормализованной экспрессии гена BIRC5 достоверно умеренно коррелировал с экспрессией его продукта – белка сурвивина (r = 0,704, p &lt;0,01) и слабо отрицательно – c экспрессией онкопротеина HER2/neu (r = –0,285, p &lt;0,05). При медиане наблюдения 40 мес общая выживаемость пациентов без экспрессии сурвивина в первичной опухоли составила 100 %, общая выживаемость пациентов с его экспрессией – 77,16 % (диапазон 74,6–80,1 %) (р = 0,041), а безрецидивная выживаемость – 100 и 71,9 % (диапазон 70,1–74,2 %) соответственно (p = 0,037). Отношение рисков прогрессирования в группе больных с экспрессией сурвивина равнялось 10,6 (95 % доверительный интервал 0,85–132,2) (p = 0,041).</p><p><bold>Заключение</bold>.
