The study included 358 patients with BC who underwent surgical treatment in breast cancer department of Academician V.I. Kulakov Research Center of Obstetrics, Gynecology, and Perinatology, Ministry of Health of Russia. Among all included into the study patients, 132 (36.9 %) had metastases in at least one axillary lymph node. Molecular genetic examination of the tumor tissue was carried out using reverse transcription polymerase chain reaction; the diagnostic panel consisted of 45 functional and 3 reference genes<bold>. </bold></p><p><bold>Results</bold>. Patients with metastases to regional lymph nodes were generally younger (p = 0.006), had larger primary tumor (p&lt;0.001) and higher total malignancy score (p&lt;0.001). The groups were also significantly different in tumor location (p = 0.005). Comparative analysis of transcriptome tumor profiling revealed statistically significant differences between groups in the level of expression of three genes: TMEM45A (p = 0.016), CCND1 (p = 0.019), and MIA (p = 0.046). Based on the data obtained we used mathematical modeling and created a predictive model, which with a high degree of probability (AUC = 0.791) allowed to predict the presence of regional lymph nodes metastases in patients with BC.</p><p><bold>Conclusion</bold>. TMEM45A, CCND1 and MIA gene expression in the primary tumor were the markers of lymph node involvement in BC. The developed predictive genetic signature can become an additional diagnostic tool to predict the risk of lymph node metastases at the point of planning the volume of axillary surgery in patients with BC.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования </bold>– выявить молекулярно-генетические предикторы метастатического поражения регионарных лимфатических узлов у больных раком молочной железы (РМЖ) на основании анализа профиля экспрессии генов первичной опухоли.</p><p><bold>Материалы и методы</bold>. В исследование включено 358 больных РМЖ, которым было проведено хирургическое лечение в условиях отделения патологии молочной железы ФГБУ «Национальный медицинский исследовательский центр акушерства, гинекологии и перинатологии им. акад. В. И. Кулакова» Минздрава России. Из общего числа пациенток, включенных в исследование, у 132 (36,9 %) были диагностированы метастазы как минимум в 1 подмышечный лимфатический узел. Молекулярно-генетическое исследование опухолевой ткани проводилось с применением методики полимеразной цепной реакции с обратной транскрипцией; диагностическая панель состояла из 45 функциональных и 3 референсных генов.</p><p><bold>Результаты</bold>. Пациентки с метастазами в регионарные лимфатические узлы чаще характеризовались более молодым возрастом (р = 0,006), большими размерами опухолевого узла (p˂ 0,001) и более высоким суммарным баллом злокачественности (p˂ 0,001). Имелись также статистически значимые различия между группами по локализации опухоли (р = 0,005). Сравнительный анализ транскриптомного профиля опухолей выявил статистически значимые отличия между группами в уровне экспрессии 3 генов: TMEM45A (p = 0,016), CCND1 (p = 0,019) и MIA (p = 0,046). С использованием математического моделирования на основании полученных данных была создана предиктивная модель, которая с высокой степенью вероятности (AUC = 0,791) позволяет предсказывать метастатическое поражение регионарных лимфатических узлов у больных РМЖ.</p><p><bold>Выводы.</bold> Экспрессия генов TMEM45A, CCND1 и MIA в первичной опухоли является маркером лимфогенного метастазирования у больных РМЖ.
