ПРАКТИЧЕСКИЕ АСПЕКТЫ КЛИНИЧЕСКОГО ПРИМЕНЕНИЯ НОВОГО КЛАССА ПРЕПАРАТОВ – ИНГИБИТОРОВ ЦИКЛИНЗАВИСИМЫХ КИНАЗ: ЭФФЕКТИВНОСТЬ И ПЕРЕНОСИМОСТЬ ЛЕЧЕНИЯ

Аннотация

Метастатический рак молочной железы остается неизлечимым заболеванием, однако в последние годы в этой области были достигнуты весьма существенные успехи, позволяющие говорить о возможности его перевода в хроническую форму, характеризующуюся длительным торпидным течением. Особенно высока вероятность достижения относительно благоприятного клинического исхода при гормонозависимом HER2отрицательном подтипе опухоли. Такой результат стал возможен благодаря разработке и внедрению в клиническую практику высокоэффективной комбинированной лечебной стратегии, связанной с появлением нового класса лекарств – ингибиторов циклинзависимых киназ (CDK) 4 и 6. В статье представлен обзор результатов рандомизированных клинических исследований представителей этого класса препаратов с данными по их эффективности и безопасности. Показано, что нейтропения является класс-специфическим нежелательным явлением и характерна для всех 3 препаратов (палбоциклиба, рибоциклиба и абемациклиба); терапия рибоциклибом, кроме того, ассоциирована с гепатои кардиотоксичностью (удлинением интервала QTc); для абемациклиба, помимо нейтропении, характерны диарея и тромбоэмболии.
